Laser Treatment for Skin of Color

Most laser complications in deeper skin tones come from the same source: a device with one wavelength, used at settings chosen for someone else's skin. The physics is not ambiguous. Melanin competes for the same light the laser is aiming at, and a wavelength that is safe on fair skin can burn, lighten or darken darker skin. The answer is not to avoid lasers. It is to use the right one at the right settings.

Why Skin Type Changes Everything

Laser treatment works by targeting something in the skin that absorbs light more strongly than the tissue around it — pigment in a brown spot, haemoglobin in a vessel, melanin in a hair follicle. The problem in deeper skin tones is that melanin is not only in the target. It is distributed through the epidermis, where it absorbs the same wavelengths. Energy meant for the follicle is taken up by the surface, and the result can be a burn, a patch of post-inflammatory darkening, or permanent lightening. The Fitzpatrick scale, which runs from I to VI, is the shorthand for how much melanin is present and therefore how much margin a given wavelength has.

What We

Treat

Unwanted hair — treated at 1064 nm in deeper skin types, where it is both effective and safe. Also the right approach for ingrown hairs and razor bumps in coarse or curly hair, which is a mechanical problem that laser treatment resolves at the source.

Pigment and uneven tone — approached carefully. Post-inflammatory hyperpigmentation and melasma are both more common in deeper skin and both worsen with aggressive treatment, so these are treated conservatively and in combination with a topical regimen.

Keloid and hypertrophic scarring — more common in deeper skin tones and assessed individually, because scar type changes what is advisable.

Acne scarring and texture — resurfacing is possible in deeper skin with adjusted settings and a longer, more cautious series.

What Makes It Safe

Four things, in order of importance:

  • Wavelength. Longer wavelengths are absorbed less by melanin and penetrate past it. 1064 nm is the safest wavelength available for hair removal in Fitzpatrick IV to VI for exactly this reason, and it is the wavelength we use for those skin types. Shorter wavelengths like 755 nm are more efficient on fair skin, and we use those where they are appropriate. Having both is the point.
  • Pulse duration. Delivering the same energy more slowly lets the surface cool between pulses, which protects the epidermis.
  • Conservative settings and test spots. Where there is any question, a small test area is treated first and reviewed before a full session.
  • A physician assessing the skin. Fitzpatrick type is a starting point, not a complete answer — tanning, medications and the condition being treated all shift the margin.

Expect

What To

Expect to be asked about your skin type and your history before anything is switched on, and expect a test spot if there is any uncertainty.

Expect a more conservative series than a fair-skinned patient would be given for the same concern. More sessions at lower settings is the trade for a much lower risk of pigment change, and it is the right trade.

And expect us to decline sometimes. Recently tanned skin, certain medications and some active conditions raise the risk enough that the honest answer is to wait or to do something else. Dr. Michael Lindley, MD, performs these treatments himself, assisted by Kathryn, RN.
