Dry Shirts Again

Excessive Sweating Treatment

Hyperhidrosis is sweating well beyond what the body needs for cooling — soaking through shirts in air conditioning, damp hands that make a handshake awkward, ruined clothing on a normal day. It is a real medical condition, it is common, and it is one of the most reliably treatable things in this clinic. In a New Orleans summer it is also badly under-reported, because everyone assumes it is just the heat.

What It Is

Sweat glands are switched on by nerve signals. In hyperhidrosis those signals fire far more than the situation requires, so the sweating is disproportionate to temperature and to exertion. It is usually focal, affecting the underarms, palms, soles or face rather than the whole body, and it typically starts in adolescence or early adulthood and continues for life if untreated. It has nothing to do with hygiene and it is not helped by trying harder with antiperspirant.

Who It

Suits

Patients who come in for this usually describe the same things:

  • Changing shirts during the workday, or keeping a spare at the office
  • Choosing clothing by how well it hides sweat rather than by what they like
  • Avoiding handshakes, or wiping a hand first
  • Sweat marks appearing in a cool room, with no exertion
  • Prescription-strength antiperspirants that did not work, or irritated the skin

If several of those are familiar, this is worth a consultation. Treatment is performed by Dr. Michael Lindley, MD.

How Treatment Works

An injectable neuromodulator blocks the nerve signal that tells the sweat gland to fire. The gland is intact and undamaged — it simply stops receiving the instruction.

  • The treatment itself takes minutes. A grid of small injections across the affected area.
  • It starts working within a few days and is usually complete by two weeks.
  • It lasts four to six months on average, often longer in the underarms than the palms.
  • There is no downtime. You go back to work afterwards.

Underarms are the most commonly treated area and the most predictable. Palms and soles respond well but the injections are more uncomfortable and numbing is used. Facial and scalp sweating can also be treated.

Before & After

Expect

What To

Expect it to work, and to work well. Of everything on this site, this is among the treatments with the most predictable result — patients are often surprised at how complete the change is.

Expect to repeat it. The effect wears off as nerve signalling recovers, so this is a maintained treatment rather than a permanent one. Most patients settle into two treatments a year, and many time the first one for late spring.

Expect some compensatory sweating elsewhere in a minority of patients — usually mild, and usually not a reason to stop.

Switched Off.

Not Endured.

Most people with hyperhidrosis have spent years working around it without knowing it has a name or a treatment. If that is you, this is a short conversation with a very good outcome.
